New Patient Registration

 

Before you arrive, please fill up the online registration form that has been sent to you by email.

 

This form is best filled out on a desktop browser.

 

Please remember to bring along the following original documents:

 

  • Identity Card/Birth Certificate/Passport/Work Permit (COMPULSORY)
  • A copy of any recent (< 6 months) dental X-rays that you may have. (RECOMMENDED)
  • A copy of any dental report that your previous dentist may have prepared.
  • A list of all the medications/TCM herbs/supplements that you are currently taking.
  • Relevant medical reports from your general physician, cardiologist, oncologist etc.
  • Dental Insurance cards/documentation and CHAS/PG/MG cards

 

Do Email us soft copies of the above in advance if possible.

 

 

Your First Visit to Elite Dental Group

 

You will receive an appointment reminder (by SMS) 1 week and 1 day in advance of your scheduled appointment.

 

Please arrive 15 mins earlier than your scheduled appointment time to complete the registration process.

 

We endeavour to see all of our patients on time, all the time. If we are running behind schedule due to unexpected delays, we will inform you as soon as possible. 

 

If you are unable to make your appointment punctually,  please let us know as soon as possible by a telephone call, so that arrangements can be made to accommodate for the lateness. Please refer to our booking policy here.

 

Also, please let us know of any special requests, preferences or concerns prior to your visit.

 

We really want to make sure your first visit with us is as comfortable as possible.

 

 

Infection control and COVID-19 Response

 

As an essential service, our practice adheres to the strictest of triaging, infection control and disinfection protocols. 

 

We are constantly on top of new government health and safety regulations as they are announced. 

 

And we are also subjected to strict audits from MOH and other relevant authorities to ensure compliance.

 

Since we were founded, we have always been practicing routine disinfection and safety protocols to comply with MOH guidelines, so as to protect you and our team from HIV, SARS, Influenza and other communicable diseases, including Covid-19.

 

As we try to adapt to new rules and added safety measures during this time,  we thank you in advance for your patience and understanding.

Dental X-rays and Dental Photography

 

Dental X-rays and high-resolution photographs of your smile and teeth will be taken at your first visit.  These are an essential part of your baseline clinical records. It is easy to miss silent dental problems if X-rays are omitted.  It may even be considered unethical for any dentist to start treatment on teeth without dental X-rays as a guide.

Special Note:  If you are Pregnant or if you can provide recent dental X-rays taken within the past 6 months, we might decide not to take new X-rays at your first visit.

 

 

Consultation, Examination and Treatment

 

A thorough examination will be done to ensure accurate documentation of any dental issues that you may have.  It is also important for us to be aware of any systemic health conditions that you may have (even if you think they may not be relevant to dentistry), since this may affect how we care for you.

 

The purpose of your first visit is NOT necessarily to perform a professional cleaning (scaling and polishing) unless there is sufficient time left over after a detailed examination and a discussion of your treatment goals.

 

At Elite Dental Group, our consultation process is unique. We aim to focus on your long term oral healthcare goals instead of just prescribing a list of procedures. This means more time spent in conversation with us during your first visit.

 

 

Pregnant Patients 

 

Tooth decay and gum disease can worsen due to hormonal changes and frequent snacking during pregnancy. 

 

Your oral health can influence the outcome of your pregnancy as well as your baby’s future health. If needed, we will work closely with your gynaecologist to ensure that you have peace of mind whilst receiving dental care during your pregnancy.

 

Please be assured that routine dentistry is generally safe during pregnancy.  It is encouraged to come in for your routine scaling & polishing at the beginning of every trimester.

 

 

Dental Emergencies

 

If your first appointment with us is for a dental emergency or acute pain,  our priority during your visit will be to provide emergency treatment and pain relief.

 

Usually there is insufficient time to provide you with comprehensive dental care, especially for complex cases.  Other non emergency dental issues will be addressed at subsequent appointments. 

 

If you have sustained a dental injury due to an accident, you may consider submitting a claim against your personal accident insurance.

 

Your attending dentist can provide a dental report ($100 before GST) to help you with the claims process.

 

 

Oral Health Tests

 

Saliva Testing

 

Saliva testing identifies risk factors (bacterial load, oral pH, inflammation level) for dental problems (such as tooth decay and gum disease). Saliva testing can make it easier to objectively measure your personal dental risk profile, optimize and assess the effectiveness of preventative measures. Saliva testing is especially useful for patients with a history of severe dental problems that have been hard to control.

 

 

Oral Cancer Screening 

 

Early cancer detection increases the chances for successful treatment. Your attending dentist will inspect and feel around your lymph nodes, tonsils, salivary glands, the perioral skin, oral mucosal lining and tongue. Any lumps, bumps, ulcers, white or red patches etc will be noted and photographed. Appropriate management usually requires specialist referral for biopsy. 

 

Oral cancer rates have been rising in patients who do not smoke or drink alcohol.

 

 

Sedation 

 

Oral sedation with medication, nitrous oxide/laughing gas sedation, IV sedation (by a visiting specialist anaesthesiologist) and lastly,  General Anaesthesia at an external Day Surgery Centre are available for adults and children with dental fear and anxiety.

 

Sedation is also a popular choice with patients who just want that extra reassurance that their treatment is comfortable, fuss-free and painless.

 

 

Medical certificates (MCs)

 

We can provide attendance slips to account for the time spent at our practice. 

 

MCs are given for other procedures that are complex and extensive (eg. Dental Implant surgery).

 

 

Cancellation and No Show Policy

 

At Elite Dental Group, we understand that there may be unforeseen circumstances that prevent you from keeping your appointment.

 

If you are unable to keep your appointment, or if you need to reschedule, please do so within 48 hours, so that other patients can be booked in to receive the treatment that they need.

Financial information

 

Your first consultation costs $180 (before GST), inclusive of any basic dental X-rays. 

 

Children’s (below 18 years old) consultation and X-rays is $150 (before GST).

 

Additional 3D X-rays (CBCT) are required for advanced dentistry (complex wisdom tooth surgery, dental implants, complex root canal treatment) and charged separately ($450 per area before GST).  Dental treatment fees will vary according to the treatment and charges apply.

 

 

Payment

 

We accept Cash, NETS, PayNow, MasterCard, Visa, Amex, Favepay, as well as immediate interbank transfers.

 

Our clinic also accepts payment from PACE, Atome and Hoolah. 

 

Interest-free instalment plans are also available to make it easier for you to say yes to the treatment that you want and need.

 

Itemised invoices are always provided. Please let us know if you prefer your invoice in a digital format.

 

 

Financial Consent

 

We will provide you with written financial documentation for your approval prior to starting any complex and extensive treatment. You will be required to initial this document. 

 

We will only proceed with treatment after you have agreed to the treatment plan and fees.

 

 

Booking Deposits

 

Each appointment is reserved exclusively for only ONE patient at a time. This allows our clinical team to deliver the highest quality dentistry.

 

Long appointments (> 60 minutes) for complex treatment are reserved with a deposit, which will come from a portion of your treatment fee.

 

Due to high demand, a booking deposit of $50 is required for ALL appointments (even those <60 minutes) that fall on Saturday.

 

Booking deposits are carried forward if you reschedule at least 48 hours prior to your appointment time. It is refunded if you cancel at least 48 hours prior to your appointment time by phone call or in person.

 

In the event of a cancellation and/or rescheduling less than 48 hours prior to your appointment, or appointment non-attendance, the booking deposit WILL NOT be refunded.

 

Please note that booking deposits are also required if you use cashless insurance. Your deposit will be refunded when you attend.

 

 

Latecomers

 

While every effort will be made to accommodate your busy schedule, please understand that we have an ethical responsibility to maintain a high standard of dental care in all circumstances.

 

If the clinical team judges that insufficient time will mean compromising on the quality of dentistry that we can deliver, then the option to carry out partial treatment will be offered to you.

 

Should you only wish to attend for complete treatment as initially planned, then we may have to reschedule your appointment.

Driving

 

There are limited basement carpark lots available in Orchard Building. The parking rate is $1.30/30 mins from 6.30am to 6pm, Monday to Friday. For Saturdays, charges are the same as weekdays. Nearby parking is available at TripleOne Somerset, 313 Somerset, Cineleisure.

 

Our clinic is wheelchair accessible.

Toys, colouring pages as well as children’s books are available for children of all ages.

 

Noise-cancelling headphones are available for use during treatment.
